Families trust us with their parents' days, and with information about their health. This page explains, in plain words, what we collect, why, and who can see it.
This is sample text for a demo website. A real center should have its policy reviewed by a lawyer and follow the Digital Personal Data Protection Act, 2023.
1. What we collect
- About the family member who registers: name, phone number, email, relationship to the elder and the portal password (stored encrypted).
- About the elder: name, age, photo, home address for pick-up, emergency contacts, food preferences and mobility needs.
- Day-to-day records: check-in and check-out times, activities joined, meals, mood and the caregiver's daily note.
- When you contact us: the details you type into our visit, contact or newsletter forms.
2. Health information
Our nurse records vitals such as blood pressure, blood sugar, pulse and weight, and notes medicines given at the center. We collect this only with the written consent of the elder or their guardian, and only to keep the elder safe during the day.
The portal shows these readings for your information. It does not diagnose anything and does not replace advice from your doctor.
3. How we use it
- To care for the elder safely and plan their day.
- To show you attendance, activity reports and vitals in the family portal.
- To contact you about your elder, including in an emergency.
- To send care tips, only if you subscribed. You can unsubscribe from any email.
We do not sell your information, and we do not use it for advertising.
4. Who can see it
- Our care team: caregivers, the nurse and the center manager, only as needed for their work.
- Family members you approve: each family account sees only its own elder.
- Doctors and hospitals: in an emergency, we share what is needed to treat the elder.
- Service providers: our hosting and SMS providers, under contracts that keep the data private.
- Authorities: only when the law requires it.
5. Photos and videos
We take photos during activities to share with families. Photos showing an elder's face appear on our website or social media only with written permission, which you can withdraw at any time. CCTV covers common areas only, never washrooms or rest rooms, and recordings are kept for 30 days.
6. How long we keep it
We keep care and health records while the elder is a member and for three years after they leave, so we can answer any questions about their care. Enquiries from people who do not join are deleted after one year.
7. Keeping it safe
The portal uses encrypted connections, passwords are never stored as plain text, and staff accounts can see only what their role needs. Paper records are kept in a locked cabinet at the center.
8. Your choices
- Ask for a copy of the information we hold about you or your elder.
- Ask us to correct anything that is wrong.
- Withdraw consent for photos or newsletters at any time.
- Ask us to delete your account when your elder leaves SilverNest.
